Thiopharm Tablet 4mg is a centrally acting muscle relaxant containing thiocolchicoside (4mg), a semi-synthetic sulfur derivative of naturally occurring colchicoside. It functions primarily as a selective competitive antagonist at central $\gamma$-aminobutyric acid ($GABA_A$) and glycinergic receptors within the spinal cord and brain stem. By binding to these inhibitory neuroreceptors, it attenuates hyperactive motor neuronal reflexes and inhibits involuntary muscle contractions without significantly compromising voluntary muscle strength or inducing generalized sedation. Additionally, it exerts mild peripheral anti-inflammatory and analgesic activities, aiding in the reduction of localized muscular stiffness, tissue swelling, and associated contractile pain.Clinically, Thiopharm 4mg is indicated for the short-term symptomatic relief of painful acute muscle spasms, low back pain, torticollis, post-traumatic or post-surgical muscular stiffness, and painful contractures associated with spinal or vertebral degenerative disorders.The standard adult dosage is 4mg to 8mg taken orally twice daily (every 12 hours) immediately after meals, with a maximum treatment duration restricted to 7 consecutive days.It is contraindicated in adolescents under 16 years of age, during pregnancy and lactation, in patients with a history of seizure disorders, and in individuals with severe hepatic dysfunction. Common side effects include drowsiness, diarrhea, abdominal distress, nausea, and mild rash.
