Sita-Met Tablet 50/500mg

Sita-Met Tablet 50/500mg is an oral fixed-dose combination antidiabetic medication containing sitagliptin (50mg) and metformin hydrochloride (500mg). It works through complementary mechanisms to optimize glycemic control. Sitagliptin, a competitive dipeptidyl peptidase-4 (DPP-4) inhibitor, prevents the degradation of active incretin hormones (GLP-1 and GIP), thereby stimulating glucose-dependent insulin secretion from pancreatic beta cells while suppressing glucagon secretion from alpha cells. Concurrently, metformin, a biguanide, reduces hepatic gluconeogenesis, decreases intestinal glucose absorption, and enhances peripheral insulin sensitivity by activating AMP-activated protein kinase (AMPK), increasing cellular glucose uptake without stimulating insulin release.Clinically, Sita-Met 50/500mg is indicated as an adjunct to diet and exercise to improve glycemic control in adult patients with type 2 diabetes mellitus when monotherapy with metformin or sitagliptin is insufficient.The standard adult starting dose is 1 tablet taken orally twice daily with meals to reduce gastrointestinal side effects associated with metformin. Tablets must be swallowed whole with water.It is contraindicated in severe renal impairment ($eGFR < 30\text{ mL/min/1.73m}^2$), acute metabolic acidosis, diabetic ketoacidosis, or known hypersensitivity. Common side effects include diarrhea, nausea, flatulence, abdominal discomfort, headache, and hypoglycemia when combined with insulin secretagogues.
