Sita-Met Tablet 50/1000mg is an oral fixed-dose combination antidiabetic medication containing sitagliptin (50mg) and metformin hydrochloride (1000mg). It works through dual complementary mechanisms to optimize glycemic control. Sitagliptin, a dipeptidyl peptidase-4 (DPP-4) inhibitor, prevents the enzymatic inactivation of incretin hormones (GLP-1 and GIP), thereby enhancing glucose-dependent insulin release from pancreatic beta cells while inhibiting glucagon secretion from alpha cells. Concurrently, metformin, a biguanide, acts via AMP-activated protein kinase (AMPK) activation to suppress hepatic gluconeogenesis, reduce intestinal glucose absorption, and enhance peripheral insulin sensitivity and muscular glucose uptake.Clinically, Sita-Met 50/1000mg is indicated as an adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes mellitus when higher metformin dosing is required alongside DPP-4 inhibition.The standard starting dose is 1 tablet taken orally twice daily with meals to minimize gastrointestinal discomfort. Tablets must be swallowed whole with water and should not be crushed or chewed.It is contraindicated in severe renal impairment ($eGFR < 30\text{ mL/min/1.73m}^2$), acute metabolic acidosis, or hypersensitivity. Common side effects include diarrhea, nausea, flatulence, abdominal pain, headache, and hypoglycemia when co-administered with sulfonylureas.
