Ranola 500mg Tablets are an extended-release antianginal formulation containing ranolazine (500mg). Unlike conventional antianginals, ranolazine acts through a distinct anti-ischemic metabolic pathway that does not rely on lowering heart rate or blood pressure. It acts as a selective inhibitor of the late inward sodium current ($I_{Na}$) in ischemic cardiac myocytes. By suppressing excessive late sodium influx, it prevents intracellular calcium overload via the sodium-calcium exchanger, decreases myocardial wall tension, improves microvascular perfusion, and optimizes myocardial oxygen supply-demand dynamics during ischemic episodes.Clinically, Ranola 500mg is indicated for the chronic management of stable angina pectoris. It is prescribed either as monotherapy or as an adjunctive treatment alongside standard regimens (such as beta-blockers, calcium channel blockers, nitrates, or ACE inhibitors) in patients with persistent angina symptoms.The standard starting dose is 500mg taken orally twice daily, which may be titrated up to 1000mg twice daily based on clinical response. Tablets must be swallowed whole with or without food and should never be broken, crushed, or chewed.It is contraindicated in patients with severe hepatic impairment, liver cirrhosis, baseline QT interval prolongation, or those concurrently taking strong CYP3A4 inhibitors (e.g., ketoconazole, clarithromycin). Common side effects include dizziness, headache, constipation, nausea, and asthenia.
