NIB 2.5mg Tablets are an oral antihypertensive formulation containing bisoprolol fumarate (2.5mg), a highly selective beta-1 (β 1 ​ ) adrenergic receptor blocker (cardioselective beta-blocker). It acts by competitively blocking β 1 ​ -adrenergic receptors located predominantly in the myocardium and renal juxtaglomerular apparatus. This blockade reduces cardiac output, decreases resting and exercise heart rates, lowers myocardial oxygen demand, and suppresses renin secretion, leading to sustained reductions in systemic blood pressure. Clinically, NIB 2.5mg is indicated for managing essential hypertension, chronic stable angina pectoris, and as an adjunct to standard therapy (such as ACE inhibitors, diuretics, and digitalis) in stable chronic heart failure with reduced ejection fraction. The typical starting dosage is 2.5mg administered orally once daily in the morning, with or without food. In chronic heart failure management, treatment begins at a low dose (often 1.25mg–2.5mg) and is titrated slowly upward based on patient tolerance and heart rate response. It is contraindicated in patients with severe bradycardia, second- or third-degree heart block, cardiogenic shock, severe bronchial asthma, or sick sinus syndrome. Common side effects include fatigue, dizziness, cold extremities, bradycardia, and mild gastrointestinal disturbances.
